Primary care physician needed for outpatient primary care practice in Northern Virginia!
Job description
We are excited to share that Annandale Family and Internal Medicine is recruiting for a Medical Doctor. This role is essential and will serve as a fulcrum for our patient care services and growth strategies.
The purpose of this position is to serve in alignment with our mission of serving our community as a trusted healthcare partner with a focus on individualizing patient care beyond the exam room with foci regarding prevention, wellness, and the whole person.
The Medical Doctor will partner closely with the leadership team including the practice physician-owner, CFO, administrative leads, as well as our vendor leaders. This collaboration will afford the ability to translate this energy from strategy into the improvement of care delivery along the spectrum of our clinic upwards to the system level.
We are seeking a physician with a passion for Value-Based Care (VBC). Specifically, the physician will serve as a curator and a primary lever for our core care programs, such as Behavioral Health Integration, Collaborative Care Model, Chronic Care Management, Remote Patient Monitoring, and our In-Home Program designed to mitigate risk such as inpatient hospital readmissions. The physician will be able to leverage these strengths to augment the efficacy, efficiency, and impact of their patient panel, with a focus on personalized patient care that has an effect on population health.
