• Credentialing Timeframe: 90 days
• General Requirements:
○ Board Certification: BC
○ License Required: MA
○ DEA: Yes
• Procedures: Upper and Lower Endos. Colonoscopies, Bread and butter GI
• Minimum Experience Required: 1 year
• Patient Population/Age: Adult
