Licensed Master Social Worker – Nursing Home Surveyor/Complaint Investigator – Syracuse, NY (#25312)

✅ Qualifications
✔️ Master’s degree in Social Work (MSW)
✔️ Active NY State LMSW license (required)
✔️ 2+ years of clinical experience in a medical facility (nursing home preferred)
✔️ Experience in nursing home surveillance preferred
✔️ Strong communication, investigative, and analytical skills
✔️ Ability to obtain SMQT certification within 12 months
✔️ Ability to travel to onsite facilities

✅ Key Responsibilities
✔️ Conduct surveys and complaint investigations in nursing homes
✔️ Review allegations of abuse, neglect, and compliance issues
✔️ Document findings and draft Statements of Deficiencies (SOD)
✔️ Monitor corrective action plans
✔️ Participate in emergency surveys and state monitoring
✔️ Testify in administrative hearings as needed
✔️ Ensure adherence to DOH schedules, policies, and procedures
