Social Worker – Admission Case Manager

Duties and responsibilities include but are not limited to:
*​Provides case management and care coordination of health care services to Veterans, including making psychosocial and psychiatric diagnoses within approved clinical privileges​
​*Links Veterans with services, resources, and opportunities, to maximize their independence, health, and well- being
​*Acts as an advocate with appropriate VA and community service providers/agencies
​*Provides professional, independent social work services to Veterans who are experiencing issues related to alcohol or substance use disorders and who are experiencing a wide range of complex medical, psychiatric, emotional, behavioral, and psychosocial problems​

Requirements:
*United States Citizenship
*Masters Degree in Social Work with fully accredited by the CSWE
*Current, full and unrestricted Social Work license from any US state or District of Columbia

*Work Schedule: Mon - Fri, 8:00am - 4:30pm
*Onsite: Role is in-person at the Grand Junction, Colorado VA Medical Center​

*Compensation: Competitive base salary range: $64,791 - $101,906. Sign-on bonus may be authorized, and access to the student loan reimbursement program if meet eligibility requirements.
