• Location: Angola, IN
• Target Start: December 30th
• Length of commitment: 3-6 months with the option to extend
• Hours: 7a-7p Call hours 7p-7a
• Current Open Schedule:
o Dec. 30-31
o Jan 2-3, 6-10, 27-31
o Feb 8-14, 17-28
o Mar 3-7
• Schedule: 3 weeks on 2 weeks off
• Type of Cases: cardiac, gen, ortho, OB, urology, GI, podiatry, healthy peds, ENT, neuro, vascular thoracic, Spinals, blocks, epidurals
• Department Description: Critical Access, 25 beds
• Number of providers during day: 3 MDs, 10 CRNAs (will supervise CRNAs)
• Number of OR's: 7a-3p - 11 sites incl. OB; 3p-5p- 5 sites incl. OB; 5p-7p - 3 sites incl. OB
• EMR: EPIC
• Requirements: Board Certified, Active IN License, DEA, BLS, Case logs required
• Travel, lodging and malpractice covered
