· Cases /Procedures: Adult and Pediatric ENT & Dental. General GI, Ortho with spine, MAC Cataract, Plastics, OB/GYN, Epidural management for C/S and PPTL, Spinal placement for all ortho and scheduled C/S, Bier blocks, Have knowledge of; will provide assistance with: Ankle blocks and IA blocks
· Board Certification: Yes
· Credentialing Timeframe: 90 days

• M-F 8a-5p, supervision model 4:1, 650-850 cases/month
