Type of cases required: Ortho, Pain Mgmt

Spinals, Blocks, Epidurals - must be proficient: Yes

Credentialing Timeframe: 60-90 days (Temps are available with a clear background / malpractice history and active, unrestricted license)

Caselogs required: Yes - proof of epidurals, spinals and blocks

EMR System: Hst E Chart

Vaccination Required: Need to be vaccinated / accepting religious / medical exemptions

Licenses: ACLS, BLS, PALS

Additional Facts Regarding Site: N/A

Must be able to work independently, sole provider at site.

Will not take Board Eligible, Must be Board Certified
