Job description

Start: September 28, 2026 (or as soon as credentialed)
Length: 13 weeks (short-term or long-term; full-time, part-time, or PRN options available)
Coverage: Scheduled visits only - no call
Schedule: Flexible Monday-Sunday, 7:00-19:00; no nights or weekends required
Call: None
Volume: 6-9 in-home evaluations per day
Practice Setting: Home health / in-home member assessments
Procedures: None

Conduct pre-scheduled in-home health evaluations for Medicare Advantage, Medicaid, and select commercial plan members
Perform medication and medical history review along with a physical evaluation in the member's home
Complete a pre-populated, patient-specific form provided for each evaluation
No prescribing, lab ordering, or changes to the member's current treatment regimen
Assignments are organized by county - candidates indicate which county they are applying for

Requirements
Board Certified - Primary Care (Geriatrics, Home Health, Hospice & Palliative Medicine, Internal Medicine, Medical-Surgical, Public Health, and Urgent Care also accepted)
New York license required, in hand
DEA not required (no prescribing or diagnosing)
New grads welcome
Credentialing timeframe: 6-8 weeks; name clear required
Orientation/training modules are unpaid (approximately 10-12 hours)

Flexible 7a-7p scheduling, Monday-Sunday
6-9 visits per day
No call, no nights, no weekends required
