Job description

Start: September 14, 2026 (or as soon as credentialed)
Length: 13 weeks (open to locums, locums-to-perm, or permanent placement)
Coverage: Outpatient clinic, scheduled shifts with no call
Schedule: Mon-Fri, 8:00-17:00 (5x8s template), full time
Call: None
Volume: Approximately 20 patients per day
Patient Population: Adults and geriatrics
Practice Setting: Outpatient clinic
Care Model: Value-based care

Outpatient primary care evaluations, chronic disease management, and preventive visits
Longitudinal follow-up within a value-based care framework
Coordination of referrals and care planning for an adult/geriatric panel
Timely documentation in Elation

Requirements
Board Certified in Family Medicine, Internal Medicine, or Primary Care
Active Colorado license or IMLC required (IMLC pathway only)
Credentialing timeline approximately 30 days
Local or Colorado-based candidates preferred; candidates requiring travel reimbursement will be considered

Mon-Fri, 8:00-17:00
No call
Interview process: NPI name clear, phone screen with recruiting leadership, final interview with regional medical director and VP of Clinic Operations
