• Join an interdisciplinary PACE (Program of All-Inclusive Care for the Elderly) team focused on delivering comprehensive, patient-centered care.
• Provide primary care services to frail and elderly populations in a collaborative, mission-driven healthcare environment.
• Work closely with physicians and interdisciplinary team members to coordinate and manage patient care.
• Conduct comprehensive participant assessments and develop individualized care plans.
• Opportunity to perform home visits to support continuity of care.
• Participate in clinical program development, quality initiatives, and interdisciplinary team meetings.
🛑 SUBMISSION REQUIREMENTS (Strict Compliance):
• Board Certified Nurse Practitioner through ANCC or AANP required.
• Active, unrestricted Nurse Practitioner license required.
• DEA Registration required.
• CDS/CSR Certification required.
• BLS Certification required.
• Minimum 5–7 years of Nurse Practitioner experience required.
• Experience working with frail or elderly populations required (or willingness to undergo training).
• Most recent NPDB Self-Query Report (within 30 days) required.
• Clean malpractice and license history highly preferred.
• Must disclose whether visa sponsorship is required.
• Not accepting fresh graduates.
• Not accepting visa candidates.
• Permanent candidates only; not accepting Locums or Locums-to-Perm candidates.
📋 CORE RESPONSIBILITIES:
• Collaborate with physicians to provide comprehensive primary care services.
• Perform initial, ongoing, and as-needed participant assessments.
• Develop, implement, and manage individualized treatment and care plans.
• Evaluate and treat acute illnesses.
• Participate in discharge planning following hospitalizations or long-term care stays.
• Conduct home visits as necessary.
• Assist with development and implementation of clinical policies and procedures.
• Participate actively in interdisciplinary team meetings.
• Provide care focused on geriatric and frail populations.
• Maintain accurate and timely clinical documentation.
• Support a compassionate, patient-centered care environment.
