Family Practice
We are seeking a skilled and compassionate healthcare professional for a locum tenens role providing high-quality in-home and telehealth care to patients. This role involves a mix of primary care services and population health management.
Key Responsibilities:
Conduct new patient visits (90 minutes) and follow-ups, assessments, and post-hospitalization care (30-60 minutes).
Perform hands-on assessments, including full vitals and head-to-toe evaluations.
Focus on medication review/management, disease education, and population health.
Coordinate with team members during POD Rounding to discuss high-risk patients and care updates.
Utilize Athena for documentation (visible in Salesforce for care manager notes) during live appointments.
Schedule & Work Environment:
Minimum 4 visits per day, with a maximum of 8.
Split schedule: 3-4 days of in-home visits, 1-2 days of telehealth.
Appointments scheduled Monday-Friday, 8:00 AM - 4:30 PM.
1-hour maximum drive between patient visits within the assigned territory.
Some evening calls may occur; return calls expected by the next business day.
Additional Details:
Call Center schedules initial visits; follow-ups are fully autonomous.
Mobile app available for visit navigation and documentation.
Occasional co-visits with team members.
No wound care or procedures required.
Qualifications:
Ability to provide exceptional patient-centered care and adapt to changing business/patient needs.
Must be comfortable with both in-home care and telehealth settings.
